If your child has never attended

Kim 4 Kids,

State Licensing requires the following paperwork be completed before you leave your child:

1. Up-to-date copy of your child's Yellow Immunization Card

2. Drop-in Care Agreement

3. Emergency Form

4. Consent for Medical Treatment

5. Parents' Rights

6. Parent Notification of Small Family Daycare

$25 per child for the entire 6 hours
or

$5 per hour per child
(minimum of 3 hours)

Saturday Afternoon Out Rates

$25 per child for the entire 6 Hours
